+ First Visit Consultation
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink 8 hours prior to your surgery. You will also need an adult to drive you home.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
If you have been booked to have surgery at the hospital, please make sure that you have nothing to eat or drink after midnight. Directions to the hospital can be found at www.durhamregional.org

| X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional radiographs are necessary, they can be taken digitally at our facility.

+ Scheduling
|
We are open Monday - Thursday from 9 am until 5 pm and Friday from 9 am until 1 pm. We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day.
We try our best to stay on schedule to minimize your waiting. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

+ Financial Information

100% of the surgery fee is due before the surgery date. For your convenience we accept Visa, MasterCard, cash and personal checks.
We also offer flexible financing through Care Credit. Qualified applicants can finance the surgery fee through Care Credit interest free.
As a courtesy we will file your insurance claim. Insurance is an agreement between you, your employer, and your insurance company. We are not a party to that agreement.

| Usual and Customary Rates:
Our practice is committed to providing the best treatment for our patients, and our fees are customary for our area. As an in-network provider for Ameritas and Delta Dental, our practice is utilizing your insurance company's fee schedule to simplify the billing process.
